New Patient Information

Alamance Eye Center welcomes all new patients! To schedule an appointment, please call 336.228.0254 or Toll Free 1.800.858.7905 between 8am and 5pm.

After scheduling your first appointment, we will send you a New Patient Packet in the mail. Please read through all the information in the packet carefully and fill out all the necessary forms so that we can process your first visit smoothly and efficiently.

Tips To Streamline Your First Visit

• Come a little early to give time for the Registration process
• Remember to bring the documents from your New Patient Packet
• You will need your Insurance Card
• Bring your photo ID
• Prepare transportation for after your visit- your eyes will most likely be dilated

Cancellation Policy

Alamance Eye Center does not charge a cancellation fee for missed appointment. We understand that there may be circumstances that will keep you from attending your appointment or may delay your arrival. Please call ahead of time if you need to cancel or reschedule your appointment, so that we can use that appointment slot for another patient. If you are running late for you appointment, we ask that you call so that we can adjust our schedule to accommodate you and our other patients more effectively.

Underage Patients

Patients under the age of 18 years old need to have a parent or guardian with them on the premises at their scheduled appointment. This policy is for the safety and well being of all our patients.
